The Community Health Worker (CHW) will be responsible for helping clients and their families to navigate and access community services, other resources, and adopt healthy behaviors. As a priority, activity will promote, maintain, and improve the health of clients and their family. Provide social support and informal counseling, advocate for individuals and community health needs. Community outreach, such as home visits will be required.
This position will work with individuals who are struggling with addiction and/or working on their sobriety. Community Health Workers can support long-term recovery by addressing these social determinants to increase access to care and improve the quality of care. They provide nonjudgmental emotional support and mentoring to individuals affected by addiction.
This position may require irregular hours. This position will also require travel by the employee in his/her own vehicle.
High School Diploma or its equivalent.
Successful completion of a Community Health Worker formal training program such as from a college or other education institution.
Verifiable good driving record and reliable transportation
Peer Recovery Support Specialists preferred or related experience. (individuals that have been successful in the recovery process and help others who are experiencing similar situations)
Job Type: Full-time, 40 hours a week.
Salary: $11.50 /hour starting out, $12.50 after completion of formal training that is paid for by TSFHS.
